Implementing Strategies for Enhanced Care and Adherence in the ICU: A Prospective Study on the PERME Scale

Objetivo

The PERME scale, widely used in physical therapy, assesses patient mobility in the intensive care unit (ICU), measuring their ability to follow commands and traverse distances within two minutes. This project aimed to implement strategies to ensure proper care of ICU patients and increase adherence to the therapeutic plan by 95% according to the PERME scale, specifically for patients with vascular, cardiac, and renal diagnoses.

Métodos

This prospective study focused on outcome, process, and balance indicators in the general adult ICU of the Complexo Hospitalar dos Estivadores in Baixada Santista, São Paulo. The ICU consisted of 17 beds, with a ratio of one physiotherapist per 10 beds/24 hours.

Resultados

From March 2022 to March 2023, a guiding diagram was developed in collaboration with ICU physiotherapists, along with conceptual changes and training on the use of the PERME scale. The scale was applied to a total of 335 patients. Adherence to the therapeutic plan based on the PERME scale reached 97%. Both individual adherence to scale usage and multidisciplinary discussions achieved 100% adherence. The percentage of patients receiving the two daily interventions was 97%. The average length of stay in the ICU was reduced to 8.6 days, and patient satisfaction reached 97.1%.

Conclusão

The utilization of improvement science methods and quality tools to map processes and identify opportunities for continuous improvement showed that monitoring indicators over time demonstrated high process reliability, ensuring the attainment and sustainability of positive outcomes.
