Preliminary insights into thrombotic and hemorrhagic prophylaxis in adults with Central Nervous System Malignant Neoplasms: A Descriptive Analysis from the TROMBOGLIO Study

Objetivo

We described the use of prophylaxis in 149 Brazilian adults with CNS malignant neoplasms diagnosed between 2021-2023.

Métodos

The TROMBOGLIO Study is an ongoing multicenter, retrospective and prospective cohort that collected data from five Brazilian hospitals. We evaluated the dependent variable prophylaxis (absence of prophylaxis, mechanical, pharmacological, or a combination of both) at postoperative and post-thrombotic event. Valid data were presented as frequency (n, %) and median (interquartile range). We conducted Chi-squared and Mann-Whitney tests.

Resultados

Among the 149 enrolled patients, 60% were middle-aged men and 93.3% received prophylaxis that frequently consisted of a combination of mechanical and pharmacological (51.7%). Only 9 (6%) participants experienced thrombotic events. Thrombotic event ratio was similar when comparing patients who received prophylaxis and those who did not (6 vs. 7.1%; p>0.050). The time between the surgical date and the initiation of prophylaxis was longer among patients with thrombotic events. Regarding pharmacological prophylaxis, the median time was 4 days (IQ 2-5) for patients with thrombotic event compared to 2 days (IQ 1-4) for those without (p=0.147). The median time for initiating mechanical prophylaxis was 3.5 days (IQ 2–5) for patients with thrombotic event and 2 days (IQ 1–3) for those without (p=0.249). Following a thrombotic event, Enoxaparin (n=6; 33.3%) and Rivaroxaban (n=5; 27.8%) were more frequently prescribed as part of the pharmacological prophylaxis.

Conclusão

Nearly 50% of the patients underwent a combined postoperative mechanical and pharmacological prophylaxis. The prescription of enoxaparin-based pharmacological prophylaxis was more common after a thrombotic event.
