The impact of covid-19 pandemic on outcomes of critically ill non-covid-19 patients: a retrospective cohort study
Objetivo
To investigate the potential impact of the COVID-19 pandemic on hospital mortality of non-COVID-19 patients admitted to four intensive care units (ICUs) in Brazil.
Métodos
This retrospective multicentric cohort study included non-COVID-19 adult patients admitted to two public and two private ICUs between January 2018 and December 2021.Comparisons were performed between patients admitted before and after the beginning of the COVID-19 pandemic (March 11th, 2020). The impact of the pandemic on hospital mortality was assessed through a multivariable logistic regression analysis adjusting for SAPS III score and type of hospital (public vs. private). Results were presented as an adjusted odds ratio (aOR) with 95% confidence interval (CI).
Resultados
A total of 21,115 patients were included in this study (10,566 patients admitted before and 10,549 during the pandemic). Patients admitted before COVID-19 pandemic were slightly older, had a similar SAPS III score, used non-invasive ventilation (13% vs. 11%; p<0.001)and vasopressors (31% vs. 28%; p<0.001) more frequently and high flow nasal cannula (2% vs. 3%; p<0.001) less frequently. They also had a lower median ICU length of stay(LOS) (2[1-5] vs. 3 [1-6]; p<0.001) and a lower ICU (8% vs. 11%; p<0.001) and hospital (11% vs. 16%; p<0.001) mortality. After adjusting the model for severity and type of ICU, non-COVID-19 patients admitted during the pandemic presented an increased chance of death compared to patients admitted before the pandemic (aOR: 1.24; 95%CI [1.13-1.35]; p<0.001).
Conclusão
The COVID-19 pandemic might have had a detrimental impact on the clinical outcomes of non-COVID-19 patients admitted to the ICUs.
