Título

Functional capacity in chronically critically ill patients 12 months after hospital discharge

Objetivo

The aim of this study is to measure hospital outcomes of chronically critically ill (CCI) patients and to evaluate 12-months survival and functional status in a sample of CCI patients selected through current definition.

Métodos

Single center historical cohort study. The sample was selected in da institutional database using the current CCI definition (ICU stay for ≥8 days added by one of the following conditions: (a) tracheostomy, (b) MV ≥96 hours, (c) sepsis, (d) severe wounds or (e) stroke (ischemic or hemorrhagic) or head trauma). Clinical characteristics, length-of-stay and hospital mortality was measured by database information’s. To analyze functional status, a convenient sample was selected based on available information of functional status before ICU stay. Functional status was measured by Barthel index.

Resultados

A total of 494 patients were selected using CCI current definition. Hospital mortality was 46%. The 12-months survival among the 55 CCI patients selected to functional follow-up after ICU discharge was 74.5%. The functional capacity in 3-6 months after ICU discharge were lower than before ICU admission and tended to recover to baseline among the survivors at 12-months.

Conclusão

The hospital mortality rate and the 12-months mortality after ICU discharge of CCI patients were high. In survivors, 12-months functional status of CCI patients decreased after ICU discharge and tended to recover to baseline at 12-months.
