Epidemiological Profile and Outcomes of Septic Patients in a Tertiary ICU: A Cohort Study

Objetivo

This study aimed to assess the epidemiological profile of septic patients admitted to a General Intensive Care Unit (ICU) in a private hospital in João Pessoa, focusing on understanding their clinical behavior to inform strategic planning.

Métodos

A cohort study was conducted in 2023, including patients aged over 18 years who met the Sepsis 3 criteria.

Resultados

A total of 337 patients were included, with 152 (45.1%) diagnosed with septic shock. The cohort was predominantly female (56.7%), with 83.4% of admissions being medical. Nosocomial sepsis was identified in 57 cases (16.9%). Non-invasive ventilation was required by 50.4% of patients within the first hour, while 5.3% required invasive ventilation, and 2.4% underwent hemodialysis. ICU mortality was 8.3%, with a Standardized Mortality Ratio (SMR) based on SAPS3 of 0.52. The median age was 72 years, with ICU and hospital stays of 3 and 6 days, respectively. SAPS3 was 49, Charlson Comorbidity Index was 1, and EPM score was 5.43. Patients who died had higher SAPS3 and EPM scores, as well as greater frailty scores. The AUC-ROC for EPM was 0.860.

Conclusão

The study identified an elderly, comorbid population with significant organ dysfunction. Higher SAPS3 and EPM scores were associated with mortality, and both scores demonstrated good predictive accuracy. The SMR of 0.52 suggests a lower-than-expected mortality rate in this population.
