Clinical and Prognostic Profile of Elderly ICU Patients: Insights from a 2023 Cohort

Objetivo

To assess the clinical and evolutionary profile of elderly patients (>65 years) admitted to the ICU.

Métodos

A cohort study involving elderly patients admitted to a private ICU in João Pessoa, Brazil, in 2023.

Resultados

Among 1417 elderly ICU patients (60.6% of total admissions), 54.5% were female, with a median age of 80 (73-87) years. Key diagnoses included dementia (34.7%), heart failure (26.1%), and previous stroke (10.1%). Clinical admissions accounted for 82.7%, with 78.2% from the Emergency Department. The median SOFA score was 3 (1-5), MFI score 0.18 (0-0.27), CCI 1 (0-3), and SAPS3 49 (38-60). Mortality was associated with higher MFI (0.37 vs. 0.18, p<0.001), CCI (3.5 vs. 1, p<0.001), and SAPS3 (71 vs. 47, p<0.001). The SAPS3 had an OR of 1.097 (95% CI 1.080-1.115, p<0.001) and an AUC-ROC of 0.867 (95% CI 0.838-0.895, p<0.001) for predicting mortality

Conclusão

The high prevalence of elderly patients in the ICU highlights the need for detailed epidemiological profiling. In this cohort, elevated comorbidity and frailty scores were significant predictors of mortality, with SAPS3 proving to be a robust prognostic tool.
