Impact of diabetes on ICU and hospital LOS following cardiac surgery with CPB: a systematic review and meta-analysis

Objetivo

The study aims to evaluate the correlation between the proportion of diabetic patients and ICU length of stay following cardiac surgery with extracorporeal circulation. Secondary objectives include analyzing the relationship between diabetes and hospital length of stay (HLOS) and assessing the impact of age, gender, and BMI on these outcomes.

Métodos

A systematic review was conducted using databases including PubMed, Scopus, Embase, and the Cochrane Library, focusing on clinical trials from 2004 to 2024. The primary outcome assessed was the correlation between diabetic patient proportion and ICU length of stay. Secondary outcomes included the correlation between diabetic proportion and HLOS, and the relationships between diabetic proportion and ICU stay with age, gender, and BMI. Weighted correlation analyses were applied.

Resultados

The review included thirteen studies with 1,588 patients. The correlation between diabetic proportion and ICU stay was weak and non-significant (-0.176, p=0.574). However, a weak but significant negative correlation was found between diabetic proportion and HLOS (-0.2545, p=0.002). Age, gender, and BMI did not significantly impact ICU stay, though significant gender differences were noted (24.98% female, 75.02% male).

Conclusão

In these study,diabetes does not significantly affect ICU stay but may lead to shorter hospital stays, an unexpected finding that requires further investigation. The study underscores the importance of considering gender as a variable in future research and suggests that age and BMI, while not significant in this analysis, remain relevant for postoperative outcomes. Further research should address these limitations to improve diabetic patient care postoperatively.
