Título

Correlation between VIS (vasoactive inotropic score) and survival rate in patients undergoing ECMO

Objetivo

This study aims to explore the impact of VIS pre-ECMO on patient outcomes, particularly examining the correlation between VIS levels and survival rates, to determine potential prognostic thresholds in critically ill patients undergoing ECMO.

Métodos

This observational, retrospective, unicenter study was performed at InCor-HCFMUSP from 2013 to 2021 to assess the relationship between VIS pre-ECMO levels and outcomes in ECMO-treated patients. Logistic regression was applied to evaluate statistical significance, focusing on identifying survival-related prognostic thresholds.

Resultados

We evaluated 160 patients on ECMO (40 VV and 120 VA), critically ill patients on inotropic and vasopressors drugs (dobutamine 126, norepinephrine 149, vasopressine 100, epinephrine 66). Logistic regression revealed a negative coefficient for VIS pre-ECMO and survival rate (-0.933) with statistical significance (p = 0.002). The study demonstrated 100% power, confirming the robustness of the findings. Notably, 70.27% of survivors had a VIS of up to 90, suggesting a significant prognostic threshold. These results emphasize the critical role of VIS monitoring in improving outcomes for patients on ECMO.

Conclusão

The analysis indicated that elevated VIS pre-ECMO levels are significantly linked to patient outcomes in the ECMO population. A prognostic threshold at a VIS of 90 was identified, underscoring the need for vigilant VIS monitoring to enhance survival rates in critically ill patients.
