Evaluation of thromboembolic outcomes in critically ill COVID-19 patients: a prospective, observational, single-center study

Objetivo

To evaluate the occurrence of thromboembolic events using rotational thromboelastometry and laboratory parameters in critically ill patients diagnosed with COVID-19 admitted to a university hospital.

Métodos

This prospective, observational, single-center study included 51 adult patients admitted to the Surgical ICU of InCor with a confirmed COVID-19 diagnosis. Demographic data, clinical conditions, and laboratory tests were recorded on the first day of inclusion and between the 3rd and 10th days of hospitalization. Viscoelastic tests were performed using rotational thromboelastometry (ROTEM) to assess clot dynamics in different coagulation pathways.

Resultados

The analysis did not show statistically significant differences in most laboratory parameters and ROTEM data between patients with and without thromboembolic events on the first day of hospitalization. However, in the second data collection, a statistically significant difference was observed in the CT Extem value, which was higher in patients with thromboembolic events (p=0.003). Hospitalization and ICU stays were shorter in the thromboembolic event group, and the need for recruitment maneuvers was significantly higher in this group (p=0.024).

Conclusão

The study identified that, while most laboratory parameters did not show significant differences, rotational thromboelastometry can reveal specific alterations associated with thromboembolic events in critically ill COVID-19 patients. Furthermore, these events are associated with shorter hospital and ICU stays but with a greater need for recruitment maneuvers. These findings may contribute to optimizing the clinical management of these patients.
